Academic Qualifications
Freshmen
Must be a high school graduate with a passing general weighted average.
Transferees
Must have no failing grades from their previous institution and a transmissible honorable dismissal.
Primary Document Checklist
PSA Birth Certificate
Original and clear photocopy.
Academic Records
Form 138 (Report Card) or an official Transcript of Records (TOR).
Good Moral Character
An official certificate issued by your previous school's principal or guidance counselor.
ID Pictures
Two recent 2×2 colored photos with a plain white background.
