General Application Requirements

 

Applicants must submit original documents to be eligible for admission.

 

1. Accomplished application form here (click to fill out the form)

2. Photocopy of Form 138 (if applicable)

3. Photocopy of PSA Authenticated Birth Certificate

4. Recommendation Form (Grade 3-10/JHS/SHS)

5. Certificate of Good Moral Character (Grade 3-10/JHS/SHS)

6. Recent digital colored photo

     1 x 1 (1 copy)

     2 x 2 (2 copies)

 

Application fee and entrance examinations are waived.

 

Admission Procedure:

 

1. Secure an application form from the Office of Admissions. Application and testing fee is Php 400.00.

2. Fill-out and submit the completed application form, together with all other requirements, at the Office of Admissions.

3. Pay the application and testing fee.

4. Secure entrance examination schedule form.

5. Take the entrance examination on the scheduled date.

6. Pay the reservation fee. Reservation fee is non-refundable but is deductible from the total fees when enrollment is made official.

 

 Entrance Examination

 

 1. Applicants must be online 30 minutes prior to scheduled examination.

2. Present entrance examination permit for examination instructions.

3. Examination results are released after five (5) working days.

 

The testing schedule is determined at the time of application. Latecomers will not be allowed to take the test; however, they may be rescheduled for testing.

General Application Requirements

 

Applicants must submit  a copy of the following documents to be eligible for admission.

 

1. Accomplished Student Application Form

2. Two (2) pcs of recent passport size photo with white background

3. Photocopy NSO Birth Certificate

4. Photocopy of NSO Certificate of Marriage, for applicants who changed their names after marriage

5. Photocopy of two (2) Government issued identification cards.

6. Photocopy of passport, for International applicants

7. A signed waiver form for those who have already booked an International English Exam.

 

Additional requirements (if applicable)

 

1. Photocopy of previous exam result, for the students who already took the exam.

2. Photocopy of PRC license, for nurses’ applicants.

3. Photocopy of Present School ID, for minor applicants

4. Photocopy of Valid ID of Parent/Guardian, for minor applicants

5. Accomplished Fetcher’s Form, for minor applicants.
