General Application Requirements

• Accomplished application form here
• Photocopy of Form 138 (if applicable)
• Photocopy of PSA Authenticated Birth Certificate
• Recommendation Form (Grade 3-10/JHS/SHS)
• Certificate of Good Moral Character (Grade 3-10/JHS/SHS)
• Recent digital colored photo 1 x 1 (1 copy) 2 x 2 (2 copies)
• Application fee and entrance examinations are waived.​​
• Applicant must submit original documents to be eligible for admission.

Admission Procedure

• Secure an application form from the Office of Admissions.
• Application and testing fee is Php 400.00.
• Fill-out and submit the completed application form, together with all other requirements, at the Office of Admissions.
• Pay the application and testing fee.
• Secure entrance examination schedule form.
• Take the entrance examination on the scheduled date.
• Pay the reservation fee. Reservation fee is non-refundable but is deductible from the total fees when enrollment is made official.
